INFORMATION REQUIRED FOR DISCLOSURE BARRING SERVICE CHECK

(BASIC DBS)

You will also need to supply (If not done so already) one piece of personal ID which includes your date of birth, such as your:

  • Passport
  • Driving Licence
  • Birth Certificate
  • National Identity card

You must also supply a copy of one of the following which is addressed to you and includes your current address dated within the last 3 months:

  • Utility bill (gas, electric or landline telephone)
  • Rental agreement/mortgage (must be typed, not handwritten)
  • Current bank/credit card statement
  • Doctor’s letter
  • HMRC formal letter

Please complete accurately using BLOCK CAPITALS. Any illegible wording will be returned to you, causing a delay in obtaining your DBS.

Authorisation of deduction from wages for BASIC Disclosure Barring Service (DBS)

...hereby agree for KSB Recruitment Consultants Ltd to apply for an

BASIC DBS and deduct £32.70

from my next wages due. Should sufficient funds not be available in my next pay week, I agree for the money to be deducted when there are.

I understand that no refund will be permitted once the DBS has been applied for, however, I do have the right to non-deduction of wages if any of the DBS hasn’t been applied for.

I am aware that KSB will post the DBS certificate to the latest address I provided them.
